I'm working on a Basic only VS.  How can I change the thumbnail from the win7 basic one to one that looks like my VS?

The icons that are there in themeui.dll so it'd apply to all of them.

I've never tried this but you could see if the BrandImage option in a .theme file will work for you: http://msdn.microsoft.com/en-us/library/windows/desktop/bb773190(v=vs.85).aspx#theme

BrandImage doesn't work.  It just adds a small icon to the lower right of the preview.  Any other ideas?

Unfortunately it's probably impossible since those thumbnails are dynamically created and are pretty much hardcoded to use bitmaps in themeui.
